About Pathfinders

Pathfinders is a new program designed to support young people with disability and their parents and carers during important life changes.

What is a big life transition? Big life transitions can be starting high school, moving between school settings, planning for employment and adulthood, or moving out of home.

These are moments in time that can be exciting but also challenging!

We help you:

  • Build knowledge, skills, and confidence.
  • Make informed decisions.
  • Feel more connected and supported.

Pathfinders bring together expert organisations and people with lived experience to offer practical, peer-led support, so you feel ready for any big life transition – no matter what it is!

The program is free for all participants. All sessions are led by people with lived experience. You can attend both in person (in Perth) and online.

Pathfinders is a collaboration between Inclusion Solutions, KIIND, and YDAN (Youth Disability Advocacy Network).
